Depending on why you’re seeing us, you’ll likely receive an examination from Dr. Khan, as well as a personalized treatment plan and cost estimate that will allow you to make the best decision for your oral health moving forward.
Patient Forms
We want to make all of your visits as simple and stress-free as possible, which is why we’ve invested in an easy-to-navigate system for patient forms. As soon as you schedule your appointment and our team adds you to our system, you’ll receive an individual link for your patient forms that will allow you to fill them out and submit them online. That way, they’re automatically attached to your patient chart by the time you come in, minimizing any waiting time.

Dr. Khan is proudly in-network with most major PPO dental insurance plans, meaning our Mount Prospect periodontal office has negotiated discounted rates and fees for patients who hold plans from certain providers. Even if we’re not in-network with your plan, our team is happy to file insurance claims on your behalf so you can receive maximum reimbursement.
